Confidentiality
Medical Records are strictly private. No information regarding your condition will be given to your employer, friends or relatives without your written consent. At your first visit, you will be given a copy of our ”Notice of Privacy Practices” that describes how medical information about you can be used and how you can get access to this information. We will ask you to sign an acknowledgement that you have received this information and place the form in your chart.
