Confidentiality
Medical Records are strictly private. No information regarding your condition will be given to your employer, friends or relatives without your written consent. At your first visit, you will be give a copy of our ”Notice of Privacy Practices” that describes how medical information about you can be used and how you can get access to this information. We will ask you to sign an acknowledgment that you have received this information and place it in your chart.
